DofE Incidents and Near Misses

The national DofE Expedition Panel would like to remind coordinators about reporting any incidents or near miss situations that happen on DofE expeditions.

It is important that these are recorded in order to monitor any trends, minimise any negative publicity and allow incidents or concerns to be followed up in an appropriate way.

The Incident-Accident Form is available for download from the Forms page.

The following guidance has been given:-

Incidents to be notified to national DofE Duty Officer:
  • Serious Injury
  • Fatality
  • Mountain Rescue Call Out
Incidents to be recorded on an incident form:
  • All of the above
  • Minor injuries (requiring visit to GP or hospitalisation)
  • Conflict with landowners or other members of the public
  • Near Miss situations (these often get reported back by participants to parents, which triggers a concern)
